[SpamCop-List] Still messed up when 'ISP has cancelled the account used to send this spam'

Three messages today, two with no potential additional reports, third
at
http://spamcop.net/sc?id=z16619335z509ed0ce62a4cd12e45c71cfee1e0202z,
refused to give a form or reporting options for the websites when the
ISP allegedly cancelled the sending account (which was at UUNet, so I
question whether there was a legitimate response for it at all).

The other bugs that affected me seem to be fixed; this one is still
there, and still needs to be stomped on.  Spam pasted in .spam,
subject "ISP CANCEL, CAN'T REPORT WEBSITES"
