Actually under ERISA the company *and* in this case the plan administrator
has an increased duty & responsibility.  Do a google search on ERISA and
insurance.

Might visit www.harp.org and look up ERISA.  You can join the associated
mail list as well.  There are some REALLY sharp people on that list when it
comes to health insurance and ERISA.

I'd do the leg work for you but I'm trying to move 3000 sq ft house full of
furniture and stuff collected over 40 years of marriage from Dallas (DFW) to
NC.
